Community Reablement and Outreach provides an island wide service to vulnerable adults in their own homes across the island. The service supports people who are moving home from hospital or who live at home and need extra support to regain or optimise their independence. Our team ‘do with’ people rather than ‘do for’ people. We are seeking two Reablement Leaders, one for our community team and one for our hospital team. The roles are similar whereby the community team member is key to accepting new referrals, undertaking initial assessments, case co-ordinating a number of people and to line manage a number of support workers. The hospital-based team member is based at St Mary’s Hospital and they will work with hospital colleagues assessing people on the wards, advising them of the services on offer and completing assessments and referrals to support people to go home or to a re-ablement bed in our resource centres. Both roles ensure a person’s reablement journey is appropriate to meet their needs. These are demanding roles that involve working a flexible roster pattern that includes weekends and bank holidays. You will be at the forefront of service delivery, leading by example and ensuring high quality services for the people we support and their families and carers.
