Community re-ablement and outreach provides an island wide service to vulnerable adults in their own homes across the island. The service supports people who are moving home from hospital or who live at home and need extra support to regain or optimise their independence. We are seeking a Re-ablement Leader to join our team who will be key to accepting new referrals, undertaking initial assessments, case co-ordinating a number of people to ensure their re-ablement journey is appropriate to meet their needs and to line manage a number of support workers. This is a demanding role that involves working a flexible roster pattern that includes weekends and  bank holidays. You will be at the forefront of service delivery, leading by example and ensuring high quality services for people using our service and their families and carers. You will need to be conversant with IT with a thorough understanding of promoting independence in a community setting.
