About the role:
We have a full time 2-year fixed term Social worker position available in our Isle of Wight Mental Health Team.
The post holder will be expected to undertake Care Act Assessments for individuals who have a diagnosis of a severe and enduring mental illness, the majority of which will be known to NHS Secondary Mental Health Services and Care Act reviews to ensure that identified outcomes are met and any identified risks are managed effectively. Other tasks will include assessments to facilitate hospital discharges, applications for s117 funding, S117 reviews, Best Interest meetings and decisions and the undertaking of Mental Capacity assessments. Attendance at CPA reviews when required, arranging MARM and safeguarding meetings when indicated. This list is not exclusive and there may be additional tasks required within the remit of this post. You will also hold a caseload allocated by the Consultant Practitioner and/or the Group Manager.
The team has a supportive and engaged management structure including a Service Manager, a Group Manager and 1 Consultant Practitioner. It also consists of other staff members, including qualified Social Workers, Adult Social Care Officers and an administration team. The Mental Health Team works alongside and supports the AMHP team.

Our Social Workers must be able to listen hard to the people of the Isle of Wight, to understand what really matters and to connect them to resources that help them continue with their chosen life, either with support or independently. You will also be working with individuals to help them to regain control of their lives in a crisis. In order to best support the people we work with, we need Social Workers who are experienced in promoting their own and others professional curiosity and who are competent in their knowledge of legislation and meeting their professional capabilities.

• Have a social work qualification and be SWE registered
• Have completed your ASYE program or have relevant statutory experience

We would welcome applications from Social Workers who have recently completed their Social Work qualification. As a Newly Qualified Social Worker you would be supported via our Assessed and Supported Year in Employment (ASYE) program. We also welcome applications from Social Workers wishing to return to practice, where you would be able to complete Social Work England requirements to regain your registration and continue your Social work journey within the Isle of Wight Council. You will share our ambitions, passion and attitude to ensure the people we serve live the best lives possible. You will be a critical thinker who has competence and confidence in your practice.
You will be able to demonstrate skills and experience of promoting a person centred and strengths-based approach. In addition, you will have proven ability to work as part of a team that puts the strengths of individuals, families and community at the heart of care and wellbeing. You will be required to take part in the Out of Hours (OOH) stand-by duty rota as and when requested by management on an occasional basis.
Here on the Isle of Wight, we are in the final year of our strategy “Care Close to Home” (CCTH). The results so far have been impressive.
Under CCTH we have: more than halved the rate of admissions into residential care amongst elders; reduced the rate of permanent admissions into care homes by working age adults by over 80%; established a highly effective and innovative approach to demand management through the voluntary and community sector (seeing less than a 3% re-referral rate into ASC); quadrupled our home based reablement offer with over 69% of people needing no onward care and support and secured the support of our colleagues across the Department in making the many changes we have so successful (over 95% of our colleagues report that they understand, support and “own” CCTH).
We are obsessed with how to best use the strengths of those we serve in designing their care and support – supporting people to regain and maintain the maximum level of independence possible. And we are blessed with having a vibrant and expert voluntary and community sector – who we regard as equal partners in the design and delivery of care and support for the people we serve (so that all of our contracts with them are now a for a minimum three years).
Of course, integration with health is a key priority – and being surrounded by water means that we really have to invest in making relationships and care pathways work effectively – as our residents cannot easily, or cheaply, go elsewhere. We have recently published our single Health and Care Plan for the IoW. Our Integrated Locality Services work effectively in avoiding attendance at A&E and “admission” (be that hospital or a care home). Equally, we are integrating care management with health across mental health and learning disabilities – and the plan is also to fully integrate commissioning budgets in these areas.
Very importantly, Care Close to Home has enabled additional investment in our workforce over the last two years – filling gaps in our previous learning and development offer and ensuring that we support colleagues to keep abreast of best practice from elsewhere and research findings by our active membership of Research in Practice for Adults. We have appointed a new Principal Social Worker (and created a new Practice Development Unit to support his work) and strengthened capacity at service manager level.
These are exciting time on the Isle of Wight within Adult Social Care, the whole service has supported a redesign that enhances strength-based practice, working with the community and our third sector partners.
