It is a really exciting time to be joining us in Community Re-ablement & Outreach as we continue to expand our service to support more people with packages of care in their own home. Our service supports people upon their discharge from hospital as well as helping to avoid people being admitted to hospital. We are seeking two Community Re-ablement Leaders to join our team.

You will be based within our team in Sandown with travel expected islandwide. Your role will be varied but rewarding. You will undertake assessments in people’s homes, and you will manage a caseload of people we support, reviewing their care needs with them however your skills and experience will help you lead, develop, motivate and support your team of support workers.

You will work closely with our Response Co-ordinators, colleagues in the Regaining Independence Team and Social Care Officers, supporting and facilitating people’s journey through the service. You will need to have experience of health and social care services along with knowledge of supporting people in their own homes with an understanding of completing care plans and risk assessments in a strengths based and person centred way.

These are demanding roles that involve working a flexible roster pattern that includes evenings, weekends and bank holidays. You will be at the forefront of service delivery, leading by example and ensuring high quality services for people using our service and their families and careers.
