You will play a vital part in undertaking the following duties:

Case coordinating people
Ensuring our clients reablement journey is appropriate for their needs
Line managing a team of support workers
Working in a fast-paced environment
Gaining strong professional relationships with the health and social care sector

For you to bring out the best in this challenging but fulfilling role, having a flexible approach to roster patterns that includes weekends and bank holidays will be essential.

You will be at the forefront of service delivery, leading by example and ensuring high quality services for people using our provision and their families and/or carers.
