Brokerage

Our brokerage team acts as a bridge between your Social Care Worker and the social care providers. We work closely with your Social Care Worker and Healthcare professionals who identify your needs and preferences. Our experienced brokers will then help them navigate the complex social care landscape, providing personalised guidance and support to access the services that best meet your requirements.

Some tasks brokers complete

• find service providers who can meet your needs and choices both on and off the Isle of Wight
• brokers match service providers with your choices and preferences
• support with discharges from hospital once assessed and medically ready to leave hospital
• provide information about service providers, for example, costs of the service, registration details or CQC ratings to your Social Care Worker/Healthcare professionals
• support Social Care Workers with information about a range of different services that could meet your needs and choices

Adult Social Care Brokerage is a service that helps individuals with assessed social care needs access the right services and resources to improve their well-being and independence.  

You will need to be below or close to the capital threshold of £23250 in savings. You can then contact the Wellbeing and Access Hub to request an Adult Social Care assessment or speak with your allocated Social Care Worker.  

We provide brokerage services to Isle of Wight residents only.
